At Sand & Sage Living, we provide person-centred in-home care for individuals who want to remain living safely and comfortably in their own homes.
We specialise in supporting people living with younger onset dementia, while also welcoming individuals with a range of support needs. Our focus is on dignity, consistency, and calm, relationship-based care, delivered at a pace that feels right for each person.
Just as importantly, we are here to support families and carers, offering reassurance, communication, and peace of mind during what can be an overwhelming journey.

Our In-Home Support Services
We offer flexible supports tailored to individual needs, including (but not limited to):
Personal Care
Dressing and grooming
Meal preparation and light cooking
Support to maintain a safe and comfortable home environment
Community & Social Access
Support to maintain interests, routines, and connection
Assistance with showering and personal hygiene
Mobility assistance and supervision
Light household duties such as tidying, laundry, and dishes
Transport to medical and allied health appointments
Café outings and lunches
Personal Care
Daily Living & Household Support
Support to maintain a safe and comfortable home environment
Support attending errands and shopping
Gentle social and community activities
